Cornell University SHP partners with Experian Employer Services to provide your 1095-B form.
New for the 2024 tax year, CUSHP members can directly access to their annual 1095-B form.
For the most flexibility, including earliest access to the form and being able to download or print it any time you need it (even after the current tax year), opt-in to electronic notification.
What if I miss the electronic notification deadline?
If you do not consent to electronic delivery by February 20, 2025, you will receive a mailed paper form for the 2024 tax year to the local address on file with Cornell University in Student Essentials or Weill Cornell Medicine's Learn Portal.
For the 2024 tax year, paper forms will be mailed on March 3, 2025, via USPS. Please allow 7-10 days for delivery.
If you opt-in to electronic notification by December 31st, you will be able to receive next year's 1095-B form electronically.
What if I need a 1095-B form for my dependents?
Any student with spouse and or child coverage during the plan year, will receive forms for those persons along with the student form.
Do I need a 1095-B form?
It depends.
In general, the Internal Revenue Service (IRS) provides some guidance on healthcare information forms. While it is not necessary to wait for your 1095-B form to file your federal income tax return, you may need the form to file your state tax return for state insurance responsibility provisions. Please check with a tax professional or state insurance department for more information.
For International Students:
We understand that international students and their families who are not receiving fellowship payments or wages will not be eligible to apply for an SSN/ITIN and therefore cannot provide a tax identification number to the university. You can still receive this form even if you do not have an SSN/ITIN.
- Students do not need to attach or submit their 1095-B form when filing federal tax returns to the IRS.
- CUSHP is required to submit this information separately on your behalf.
- If you are filing state taxes, you may need the 1095-B form for filing your state tax return.
What if I don't do anything or don't want a form?
In compliance with the Affordable Care Act (ACA), the Cornell University Student Health Plan (CUSHP) submits completed 1095-B forms to the Internal Revenue Service (IRS) on behalf of all individuals who were enrolled in SHP during that calendar year. This submission verifies that SHP members had insurance that provides minimum essential coverage.
CUSHP is required to provide you with this document because we use your taxpayer identification number when we submit the completed 1095-B form.
What information is used to create a 1095-B form?
CUSHP reports to the Internal Revenue Service (IRS), the taxpayer identification number, either a Social Security Number (SSN) or Individual Tax ID Number (ITIN), and the months in which a member had SHP insurance during a calendar year to create the 1095-B form.
Please make sure that your University Registrar has your SSN or ITIN on record. For Cornell Ithaca and Cornell Tech students, log in to your Student Essentials account to check that your SSN/ITIN number is listed correctly. For WCM students, please log into your LEARN portal.
If you have not yet provided your Registrar with your SSN/ITIN, please do so by December 31 using one of the following methods:
- In Person – When visiting one of the below offices, please bring a copy of your original social security card or taxpayer/ITIN letter and two forms of identification.
- For Cornell Ithaca and Cornell Tech: Visit the Office of the University Registrar, located at 245 Day Hall, Ithaca, NY.
- For Weill Cornell Medicine: Visit the Office of Student Accounting, located at 1300 York Avenue, Room C-114, New York, NY.

- Save your photo in PDF format.

What if I need to replace my 2024 form?
If you opted-in to electronic delivery for the tax year that just ended:
- Login to the SHP 1095-B site using your NetID or CWID.
- Select "Year-End Statements" and access "Current Year End Statement".
- Select the "2024 Form"
- Click download
If you did not opt into electronic delivery, you can get a one-time, timed access (10 minutes) to a digital re-issue of your 1095-B form, or you can have another form mailed to you.
- Login to the SHP 1095-B site using your NetID or CWID.
- Select "Year-End Statements" and access "Current Year End Statement".
- Select 2024 Form.
- Choose if you would like to receive a digital version or a mailed form.

What if I need a form from 2023 or earlier?
If you need an electronic copy of your 1095-b form for a tax year prior to 2024, please send an email to shp-finance@cornell.edu. Please include your full name, 7-Digit Cornell or Weill Cornell Medicine ID, and enter "1095-B" in the subject line.
