How to apply for a continuation of SHP coverage
Application and payment must occur within 60 days of your plan expiration date.
To apply for a one-time 90-day continuation of coverage:
- Complete application form.
- Pay for continuation of coverage.
- Submit completed application form and payment receipt to Gallagher Student Health.
You cannot continue your SHP coverage if you do not submit both the application and receipt within 60 days after the later of either the initial SHP coverage expires or receive notification of your SHP coverage expiring.
Please allow 3 business days for Gallagher Student Health to process your continuation of SHP coverage request and another 3 business to activate your enrollment with Aetna and OptumRx.
There are no refunds on the continuation of coverage on or after the start date of continuation. Please see the SHP certificate of coverage (“Continuation of Coverage” information) for details.
