How to Apply for SHP Continuation

Application and payment must occur within 60 days of your plan expiration date.

To apply for a one-time, 90-day continuation of coverage:

  1. Complete the SHP continuation of coverage application form. 
  2. Pay for the SHP continuation of coverage through the payment portal. A Cornell NetID and password are required to access the portal.
  3. Email your completed application form and PDF payment receipt to Gallagher Student Health at Quincy.BSD.enrollmentteam@AJG.com. Screenshots of the payment confirmation will not be accepted.

You must submit both your completed application form and payment receipt within 60 days of whichever happens later:

• Your initial SHP insurance expires, or
• You receive notice that your SHP insurance is expiring.

If you do not submit both documents within that 60-day period, you cannot continue your SHP insurance coverage.

Please allow 3 business days for Gallagher Student Health to process your continuation of SHP coverage request and another 3 business to activate your enrollment with Aetna and OptumRx.

There are no refunds on the continuation of coverage on or after the start date of continuation. Please see the SHP certificate of coverage (“Continuation of Coverage” information) for details.
