Students Going on a Leave of Absence

Student Health Plan (SHP) and Student Health Plan Plus (SHP+) members who go on an approved Leave of Absence (LOA) during the academic year have the option to maintain, cancel, or extend their insurance coverage, depending on the date of their LOA. 

Please see below for instructions for cancelling or extending your coverage. 

Options for students taking a LOA

As a student on leave, you are eligible to maintain your SHP coverage. Please refer to the below choices associated with your leave date.

Students who take a LOA between 8/31 & 1/31 may choose to:

  • Maintain your SHP/SHP+ coverage through the end of the fall semester (through 1/31) [default if you do not take action]
  • Purchase SHP coverage for three months beyond your fall-semester coverage (from 2/1–4/30)
  • Purchase SHP coverage through the end of the plan year (through 7/31/21) 
  • Cancel your SHP/SHP+ coverage without a cancellation fee

Students who take a LOA between 1/1 & 1/31 may choose to:

  • Accept the automatic cancellation of your spring-semester SHP/SHP+ coverage [default if you do not take action]
  • Purchase SHP coverage for three months beyond your fall-semester coverage (from 2/1–4/30)
  • Purchase SHP coverage through the end of the plan year (through 7/31/21) 
  • Cancel your SHP/SHP+ coverage without a cancellation fee

Students who take a LOA between 2/1 and 3/31 may choose to:

  • Maintain your SHP/SHP+ coverage through the end of the plan year (7/31/21) [default if you do not take action]
  • Purchase SHP coverage for three months beyond your fall-semester coverage (from 2/1–4/30)
  • Cancel your SHP/SHP+ coverage without a cancellation fee 

Students who take a LOA between 4/1 and 7/31 may choose to: 

  • Maintain your SHP/SHP+ coverage through the end of the plan year (7/31/21) [default if you do not take action]
  • Purchase SHP coverage for three months into the next plan year (through 10/31))

How to purchase extended SHP coverage

Complete a SHP Extension Enrollment Form by logging in to Gallagher Student Health with your Cornell NetID and password. The form is located in the Plan Details section:

  1. Click Other Forms
  2. Click Plan year 2020 – 2021
  3. Download Extension Enrollment Form
  4. On the form, in the Reason for request, indicate “Other” and write Leave of Absence”
  5. To be enrolled for the Spring semester, indicate 6-month extension under the fall term
  6. To be enrolled through April 30th, check the “Other” term box

Payment: Payment for either option can be charged to your Bursar if you complete the extension form within two weeks of your status change. Otherwise, payment must be made directly to: Cornell Health, Attn. Robert Kilts, 110 Ho Plaza, Ithaca, NY 14853.

How to cancel your SHP/SHP+ coverage

(Not available to students taking a LOA after April 1)

If you wish to cancel your SHP/SHP+ coverage, complete a Cancellation Form by logging in to Gallagher Student Health with your Cornell NetID and password. The form is located in the Plan Details section:

  1. Click Other Forms
  2. Click Plan year 2020 – 2021
  3. Download Cancellation Form
  4. When completing your form, indicate Leave of Absence (LOA) as your cancellation reason

If you elect to cancel your plan, your coverage will end on the last day of the month you submit your cancellation request. Once the cancellation is processed, SHP members will receive a prorated refund of the SHP premium.
