SHP+ Claims Submissions
Aetna participating providers
As a Student Health Plan Plus (SHP+) member, whenever you receive care at an Aetna Participating Provider (including Cornell Health), any claims that need to be submitted for reimbursement will be submitted directly to Aetna for you.
When a claim is submitted on your behalf, you will receive and Explanation of Benefits (EOB) in the mail after your visit that explains what your provider charged and what was covered under SHP+.
If you receive care from a non-participating provider, and you want to submit a claim for reimbursement, you will be responsible for submitting the claim yourself.

Complete sections 1-28. Sections 29-45 need to be completed by your provider, or you may attach an itemized bill from your provider.
Send the completed form, along with your bill, to Aetna Student Health:

- Online: through Aetna Navigator, Aetna's secure member's portal (registration required)
